On-site help with applications now in Sandy!

The Healthy Kids program is an effort to provide all Oregon children with health coverage. It is not only for those families at or near the poverty line, but also to help families of many income levels who have difficulty obtaining or affording health insurance for their children.

Information on this important program is available on the website: www.oregonhealthykids.gov Applications can be made online at that site.

For personal help completing your application: Sandy area volunteers who have taken the training and become registered “Healthy Kids Assistors” are available. They have arranged with the Sandy Community/Senior Center (38982 Pioneer Blvd.) to staff two shifts of regular hours at the Center to help families with their applications. Those shifts are:
Tuesdays – 4:00 to 6:00 PM
Saturdays – 12 (Noon) to 3:00 PM

They have the applications and can submit the completed forms for those applying. Applicants need to bring income records, birth certificates, social security information, and proof of legal residency in Oregon. Copies of documents can be made on site to submit with applications.
Contact – Cathy Fantz (503-637-6495)

Note: These volunteers cannot qualify applicants, only assist with submitting the required forms and documents.
